Primary Home Care is a non-technical, medically-related personal care service provided to eligible Medicaid clients whose health problems cause them to be functionally limited in performing activities of daily living.
These services are prescribed by Physician's as a part of client's plan of care and are provided by a Primary Home Care Attendants.
Eligibility for these services is determined by the verification of the applicant's Medicaid status by the Texas Department of Aging and Disability Services.
Home Health Attendants will help you with your Activities of Daily Life (ADL's) and Independent Activities of Daily Living (iADL's) as determined by your physician. Your attendant will provide you non-skilled, non-technical services in the comfort of your own home.
